21 years of age or older
Resident of the State of TN
Adequate Social Support (24 hr. caregiver or *If client can utilize PERS, only 16 hours of caregiver are required. Eight (8) hours will be monitored by PERS)
Safe Home Environment
Must NOT be enrolled in another Statewide (TN) Waiver Program
